Rhinoplasty: Cosmetic and Reconstructive

In nasal surgery, there has been a recent trend to perform "open"rhinoplasty. The "open" rhinoplasty approach places a skin incision along the bottom portion of the nose. This is not only contradictory to our philosophy at Otolaryngology-Facial Plastic Surgery of Long Island but "bucking" the evolving trends in minimally invasive facial cosmetic surgery. Our practice depends and uses "closed" techniques with small endoscopes so the nose can be reconstructed and aesthetically balanced in quicker time. There are no external scars to worry about. Much better visualization is afforded using telescopes instead of peeling open the nose which takers longer to perform and heal.

Endoscopic Facial Plastic Surgery (EFPS)

An endoscopic face lift is an alternative to traditional face lifts. An endoscope is a probe with a camera attachment that beams the image to a television screen so that the surgeon can see what's underneath the skin. An endoscopic surgery requires smaller incisions than traditional face lift surgery. These smaller incisions reduce the number of complications, speeds up the recovery time, and are easier to hide.

The results are not as dramatic as a traditional face lift, but it's perfect for people who are just looking for a maintenance procedure to cure initial signs of aging. For younger people it's an ideal procedure because it is a subtle change that is difficult to detect, and can give the impression of perpetual youth.

Laser Surgery

Lasers produce a beam of light in a single wavelength with variable intensity and pulse rate that is capable of precisely cutting, sealing or vaporizing tissue such as skin and blood vessels. Laser light is absorbed by water or dark pigments (melanin in skin, hemoglobin in blood) in the tissue and converted to heat. A variety of functional and cosmetic skin problems are treatable with lasers, which are often specialized to handle a particular type of problem (there is no "cure-all" laser capable of correcting all problems).

Lasers offer many advantages over traditional, mechanical technology, including replacement of scalpels, reduced risk of infection, "bloodless" surgery, less scarring, high precision, limited injury, and increased speed of procedures. Results may even be better with lasers.

Incisionless Eyelid Surgery

This procedure is performed on the lower eyelid only in properly selected patients. Most patients today will receive this type of approach to their lower eyelid for removal of fat pads (baggy lower lids). A small opening is made inside the eyelid through the conjunctiva (Transconjuntival) to remove the excess fat.

Neck Recontouring

Otolaryngology-Facial Plastic Surgery of Long Island has introduced a new technique for neck recontouring by combining two existing well-known modalities. This new technique uses an endoscope and a soft tissue shaver. The endoscope enhances visualization through a small incision while the soft tissue shaver atraumatically removes neck fat.

Neck recontouring will be revolutionized by this new technique described and utilized exclusively by Dr. Schaeffer and Dr. Ephrat. We believe this technique is less traumatic and more accurate than current techniques. Standard fat extraction by liposuction depends on external cues of visualization and palpation to determine the end point of fat extraction. The endoscope by virtue of its better visualization can directly contour fat and its boundaries.
